Before your organization's Challenge Discovery day, each participant will need to provide us with a signed Assumption of Risks form (below) as well as a completed Medical Information form (below). Participants must return both forms to a designated program coordinator at their office. (If you are concerned about your privacy, please inform your program coordinator that you would prefer to fax the forms directly to the Challenge Discovery office at (866) 448-9385.)

REQUIRED FORMS

Assumption of Risks. This form describes the nature of the risks associated with programs and activities at Challenge Discovery. Please remember that signing this form does not require participation in any activity; we simply want participants to be informed of any risks associated with their participation. (Participants are asked to read it carefully; it is a legal document.)

Medical Information. This form should be completed by every participant to inform us of any medical history, condition or medication that we should be aware of before their participation.
